Truthful trend intelligence

Every visit looked fine.
The decline didn't.

Veritrend reads across every visit — captured hands-free by a small wearable pendant — and surfaces the slow changes — eating less, drinking less, more pain, less steady on their feet — that no single note can show. So your team can act before a fall, a UTI or an admission, not after.

The gap

Your notes record what happened. They can't show what's changing.

Events versus trends. With a different carer most days, no one sees the whole line.

On an hour-long visit, the telling detail from the first ten minutes — how they slept, whether they wanted that drink — is gone before anyone writes it up. Today those trends live in people's heads as a hunch at a monthly review.

What a single visit shows

"Made breakfast. Ate about half. Took medication. Watched the news together. Walked to the bathroom unaided. Seemed in good spirits."

Reads fine. Files cleanly. Nothing to flag.

What Veritrend sees across the fortnight

  • Intake poor 6 of last 8 visits
  • Fluids below threshold five days running
  • Steadiness slipping — two near-misses logged

How capture works

A pendant, not a phone.

Everything Veritrend knows starts with a small wearable pendant on a lanyard. The carer presses it once on the way in and once on the way out — that's the entire interaction. No phone out mid-conversation, no typing in front of the client, no forms at the doorstep. A light shows when it's listening, so consent is visible, not buried in paperwork. The carer just talks — to the client, through the visit, the way good care already sounds. The pendant turns that conversation into the record.

Consent, visibly

A recording light shows when it's on. Client and family consent is captured before any visit.

Nothing stored on the device

Audio is encrypted, transferred and wiped — raw audio is deleted once the note is structured.

Built for real shifts

Hands stay free, eyes stay on the person — designed with carers, refined in pilot.

The pendant is how the data exists. Reading it across every visit is what no one else does.

What it watches

Seven quiet signals that precede a crisis.

Nutrition

Intake falling before weight loss shows.

Hydration

Sustained low fluids — precursor to UTI, confusion and admission.

Pain

Rising pain, or relief that's stopped working.

Mobility & falls

Growing unsteadiness or near-misses — early falls signal.

Skin integrity

A new or worsening concern, tracked from first appearance.

Mood & wellbeing

A sustained dip in mood — a wellbeing and safeguarding sentinel.

Continence

Days without a recorded movement — a constipation risk left invisible.

Also

…and it listens for actions too — say "raise an incident" during a visit and a pre-filled incident form is waiting for sign-off. Patterns are surfaced for team review, never treated as clinical decisions.

None of this asks carers to do anything new — the pendant hears what they already observe, and Veritrend does the analysis.

CQC evidence

The gap between Good and Outstanding is the gap between recording and preventing.

A complete record of events holds you at Good. Spotting decline early — and acting on it — is the Outstanding story. You can't show that from event logs, only from trends.

Aligned to the CQC Single Assessment Framework, built to survive its coming changes. It's a readiness and evidence tool — never predicts or replaces an inspector's judgement.

Why it pays

It doesn't just save time. It helps you win more work.

Freed admin time isn't only a cost saving — in today's commissioning it becomes more chargeable capacity from the same team, and a stronger hand when work is allocated.

Capacity

More capacity, more packages

Councils route new packages to whoever can say yes now — take on more visits without hiring.

Rating

A rating that opens doors

Many frameworks only accept Good or above and reward quality; live trend evidence strengthens the rating.

Outcomes

Evidence the outcomes they now buy

Commissioning is shifting from time-and-task to outcomes; trend data proves reablement results like safe, earlier discharge.

Future-ready

Ahead of the digital rules

Commissioners are writing care technology and digital records into specs — have it already.

Allocation rules, tiers, zones and fee models vary by council.

Intelligence layer, not another system

It works with the care system you already have.

Not a care-planning system to migrate to. Not another AI scribe. The layer on top that reads visit data out of existing software and writes the confirmed note back.

Because confirmed changes write straight back, the care plan keeps up in real time — barely drank last night, tomorrow morning's plan already says so. Genuinely living, not reviewed twice a year.

"AI note-takers help you write the note. Veritrend is the only part that reads across all of them."
